ISMP Medication Safety Self Assessment® for Perioperative Settings: How to Obtain the Most Valuable, Accurate, and Useful Results
The perioperative setting is one of the most medication-intensive areas in healthcare, often with more medications administered per patient than in any other patient care setting. The limited use of what is known to be typical safety checks and balances in other settings during the medication use process, along with the common use of high-alert medications, time-sensitive tasks, and complex and stressful working conditions contribute to making the perioperative area particularly error-prone.
The Institute for Safe Medication Practices (ISMP) was awarded a grant from the US Food and Drug Administration (FDA) to create a self-assessment tool designed to evaluate the safety of medication use in perioperative settings across US hospitals and freestanding ambulatory surgery centers (ASCs). This self-assessment tool is designed to heighten healthcare providers’ awareness of best practices associated with safe medication systems, assist healthcare providers with identifying and prioritizing gaps in perioperative medication systems to avoid patient harm, analyze the current state of medication safety in perioperative settings, and create a baseline measure of national perioperative medication safety efforts. Like other ISMP assessment tools, participating organizations who submit their findings to ISMP will have access to aggregate preliminary data to compare themselves to demographically similar hospitals and ASCs, and to aid in prioritization and development of a facility-specific action plan.
