Medication Error Reporting
FOR HEALTHCARE PROFESSIONALS
Report an actual or potential medication error, near miss, or hazardous condition through the ISMP National Medication Errors Reporting Program (ISMP MERP). Healthcare professionals can confidentially share medication-related events and concerns so ISMP can identify contributing factors, emerging risks, and opportunities to improve medication safety.
Vaccine Error Reporting
FOR HEALTHCARE PROFESSIONALS
Report an actual or potential vaccine-related error or hazardous condition through the ISMP National Vaccine Errors Reporting Program (ISMP VERP). Reports help ISMP identify risks associated with vaccine prescribing, preparation, dispensing, administration, storage, and other parts of the medication-use process.
Consumer Error Reporting
FOR PATIENTS, FAMILIES, AND CAREGIVERS
Did you or someone you care for experience a medication error or discover a potentially unsafe medication situation? Consumers can report medication concerns directly to ISMP. The information you provide helps identify safety issues and opportunities to prevent similar events from affecting other patients.
California Medication Error Reporting
FOR CALIFORNIA COMMUNITY PHARMACIES
The Institute for Safe Medication Practices is the entity approved by the California Board of Pharmacy to receive medication error reports from community pharmacies under California Business and Professions Code Section 4113.1. Community pharmacies can access the California Medication Error Reporting (CAMER) system to register and submit required reports.
ECRI and ISMP Patient Safety Organization Reporting
FOR HEALTHCARE PROFESSIONALS AND ORGANIZATIONS
Healthcare practitioners may choose to submit medication or vaccine events to the ECRI and the Institute for Safe Medication Practices Patient Safety Organization (PSO). Certain patient safety information prepared within an organization's patient safety evaluation system and submitted to the PSO may qualify for federal confidentiality and privilege protections under the Patient Safety and Quality Improvement Act.

Medical Device & Clinical AI Reporting
FOR HEALTHCARE PROFESSIONALS AND ORGANIZATIONS
Report a problem, hazard, or safety concern involving a medical device or clinical artificial intelligence technology. ECRI reviews reported problems and may investigate the concern, communicate with manufacturers, identify broader hazards, and share important findings with the healthcare community.
Vendor Safety Notice
FOR HEALTHCARE FACILITIES, PROVIDERS AND PROFESSIONALS
Submit a notice received from a manufacturer or vendor regarding a recall, corrective action, product problem, or other safety concern. ECRI may use submitted notices as source material when evaluating emerging risks. Submitter-identifiable information is not displayed.
